Submarine Veterans Vests

In order to be as accurate as possible, please input the text blocks with upper case and /or lower letters as you wish, and ensure the entries are the the order you wish from top to bottom. Check Spelling.

VEST SIZE

Select from the following: S, M, L, XL, XXL, XXXL

Note: The vests are sized rather small. Recommend selecting your shirt size or one size larger.

A. NAME, LIFE MEMBER, HOLLAND CLUB MEMBER, NICK NAME, ETC (3 line limit, any order)

B. Reserved for: BREMERTON BASE PATCH or USSVI LOGO PATCH

C. OPTIONAL PERSONALIZATION AREA: Use for CO, COB or other specialty areas

D.. OPTIONAL PERSONALIZATION AREA: Used primarily for LAST NAME

E. Reserved for Large USSVI Logo with BREMERTON BASE embroidered over

Specify Silver or Gold Dolphins for Logo. SILVER GOLD

F. OPTIONAL PERSONALIZATION AREA: Used for ASSOCIATE MEMBER, STATE, etc

G. BOAT LIST: NAME, TYPE & HULL NUMBER

There are 4 or more methods for getting the completed form to the storekeeper. Preferred in the following order:

1. Submit by clicking submit button and it will be e-mailed to him.

2. When filled in, copy and paste the entire form into the text area of a e-mail message and send to rwharris@wavecable.com

3. Print by (Push PRINT icon on browser when form complete). and snail mail it to Ralph Harris, 3131 SE Forest Villa Ct. Port Orchard WA 98366-0175
